Nurse Practitioner (Skilled Nursing Facilities)

About the Role

We are partnering with a premier healthcare provider to find compassionate and skilled Nurse Practitioners to deliver primary care services within skilled nursing facilities across New York City. You will be a critical part of a dedicated clinical team, managing patient care, conducting assessments, and collaborating with physicians and facility staff.

Key Responsibilities

• •Provide primary and preventative care to residents in a skilled nursing facility setting
• •Conduct physical examinations, diagnose, and treat acute and chronic conditions
• •Develop and implement patient care plans in collaboration with an interdisciplinary team

Qualifications

• •Current license as a Nurse Practitioner (NP) in the state of New York
• •Board certification as an FNP, ANP, or GNP
• •Experience in geriatric care, long-term care, or skilled nursing facilities is highly preferred
